Please collect the Forms from me or download from the following link :

Please note the following Copies which must be submitted with the Form :

1) Copies of educational Documents ( 1 set )

2) Copy of Punjab University Registration Card ( B.A/BSc/B.Com/MA/MSc ) ( 1 set )

(Student who do not posses Registration Cards , Please Contact Hafiz Abrar of IAS )

3) Copy of CNIC (1 set )

4) Passport size Photographs ( 2 Photo )

Please Note the Following Instructions in Filling the Form :

1) Please do NOT fill and LEAVE the Following Section Empty ;

2) In Section 13 , Only Fill The Signature of the candidate and Contact Number

Please submit the form ASAP , along with the

Submission of form will accelerate the Distribution of Degree
